Newly-enrolled international students studying at this College shall receive health examinations at the Wuxi Health Center for International Travel.

I. Required Materials

1. ID Documents:

(1) Aliens: The original and 1 photocopy of passport (photo page + visa page/visa exemption page + information change page).

(2) Hong Kong and Macao Residents: The original and 1 photocopy of Mainland Travel Permit for Hong Kong and Macao Residents (photo page + visa page/visa exemption page + information change page).

(3) Taiwan Residents: The original and 1 photocopy of Mainland Travel Permit for Taiwan Residents (photo page + visa page/visa exemption page + information change page).

2. Three identical 2-inch bareheaded identification photos taken in 6 months.

3. If an overseas health examination record is available, please bring the health examination record issued by a hospital designated by the Embassy where the student is located for reference, including the original medical reports and original X-ray films.

II. Notice for Health Examination

1. Fasting is required on the day of the health examination. Maintain a light diet for three days before the health examination;

2. Please inform the healthcare provider in advance if you are pregnant or may have conceived, in which case, an X-ray examination is not required.

3. Do not wear metal accessories when receiving the X-ray examination.

4. For females, the menstruation period should be avoided;

5. Currently, payment can only be made in RMB and cash;

6. In case of needing vaccinations, please refer to the vaccination menu.

III. Review

1. In case of any abnormalities are found in the health examinations and a review is required, please visit the Center for review within the specified time with your ID documents as required by the notification.

2. The issuance of the certificate will be determined based on the results of the review or further examination.

IV. Certificate Collection

1. The certificate shall be collected by the examinee (or agent). If the health examination is qualified, the examinee (or agent) shall collect the certificate within the specified time at the certificate issuance window with the Certificate Collection Slip (on which the collection time is indicated).

Health examinations are not offered in the afternoon.
